Summary

Increasing Diabetes Self -Management Education in Tribal Settings Report (PDF)

This report details the Building Community Supports for Diabetes Care project implemented through the Montana-Wyoming Tribal Leaders Council. It describes the Tribal-Academic-IHS partnership that developed and implemented a community-based Diabetes Self-Management Education (DSME) Program in two tribes. The program involved training Tribal staff who had little prior knowledge of DSME and relied on Tribal leadership for implementation. The report includes program evaluation results, impacts on knowledge and attitudes about diabetes and changes in participant clinical measures. Finally, it presents lessons learned that will be helpful to other Tribal communities interested in addressing diabetes self management.
