March 2004: Collaborative Learning Network
The Robert Wood Johnson Foundation Diabetes Initiative held its third Collaborative Learning Network (CLN) Meeting in St. Louis, MO on March 3 - 4, 2004. In attendance at the CLN Meeting were the Diabetes Initiative Grantees for both the Advancing Diabetes Self Management and the Building Community Supports for Diabetes Care programs, the National Program Office Staff, a member of the National Advisory Committee, a member of the RWJF Program Staff, and Invited Experts in the field. The overall purpose of this meeting was to: 1) Present Key Topics in Diabetes; 2) Allow for ample grantee workgroup time; and 3) Discuss grantee improvement plans for linking interventions with intended audience.

July 2004: Second Annual RWJF Diabetes Initiative Meeting
The Robert Wood Johnson Foundation Diabetes Initiative held its second Annual Meeting, entitled Diabetes Self Management: The Bridge to Better Health, in San Francisco, CA July 14 - 16, 2004. Keynote speakers Dr. Felix Aguilar and Amy Friedman discussed clinic-community collaborations and Dr. Kate Lorig lead a discussion and exercises on developing key messages and promoting self efficacy for self management. During a Poster Viewing and Networking session on the evening of July 14th grantees had the opportunity to display and share their work. Sharing continued throughout the meeting with a grantee sessions called In the Spotlight. Holyoke Heath Center, Inc., MaineGeneral Health, Minneapolis American Indian Center, and Providence St. Peter Family Practice Residency presented aspects of their project. Other highlights included a socio-drama presentation by promotoras from La Clinica de La Raza-Fruitvale Health Project, Inc. and invited speaker presentations on a variety of topics such as project sustainability. In attendance at the Annual Meeting were the Diabetes Initiative Grantees for both the Advancing Diabetes Self Management and the Building Community Supports for Diabetes Care programs, National Program Office Staff, the National Advisory Committee, RWJF Program Staff, the Research Triangle Institute International External Evaluation team, and invited experts.

August 2004: Chronic Disease Self Management Program Training
The National Program Office of The Robert Wood Johnson Foundation Diabetes Initiative hosted a Chronic Disease Self Management Program Master Training in Chicago, IL on August 17 - 20, 2004. Teams from nine Diabetes Initiative grantee sites and the National Program Office completed the training to offer workshops and train other leaders in their respective locations. There will be a follow up discussion on implementation strategies at the fall Collaborative Learning Network meeting in St. Louis.
November 2004: Collaborative Learning Network
The Robert Wood Johnson Foundation Diabetes Initiative held its fourth Collaborative Learning Network (CLN) Meeting in Las Vegas, NV on November 16 - 17, 2004. In attendance were the Diabetes Initiative Grantees, Program Staff, members of the National Advisory Committee, a member of RTI International Evaluation Staff, and invited speakers. Dr. John Foreyt and Dr. Debra Haire-Joshu discussed weight management in diabetes focusing on lifestyle interventions and environmental and policy support for weight management. Grantees from Campesinos Sin Fronteras, Metro Denver Black Church Initiative, and Gateway Community Health Center gave "In the Spotlight" presentations on lessons learned and models used with their programs. Grantees also prepared poster presentations on the topics of culture or a significant program highlight from the first 6 months of their implementation phase.
