Medical Assistant Confidence Level Survey (PDF)

This one-page assessment tool is designed to be completed by medical assistants to assess their level of confidence in performing diabetes self management activities with patients. This tool was developed in a program that includes medical assistants as critical members of the diabetes provider team.
