Full Circle Diabetes Program Resource Toolkit: Your Guide to Successful Program Implementation (PDF)

This 61-page tool was developed by the Minneapolis American Indian Center to be a resource toolkit for developing holistic program interventions to build community supports for diabetes care in the community. The resource describes the processes used in Minneapolis to develop their diabetes program centered around community needs and interests.
