SONRISA: A Curriculum Toolbox for Promotores / Community Health Workers to Address Mental / Emotional Health Issues Associated with Diabetes (PDF)

SONRISA is a resource for community health workers to learn about depression and other emotional health issues associated with diabetes, to work with clients who are distressed, and to identify those who are appropriate for referral to a health care provider for further evaluation and treatment.  The tools can be used individually or in combination to develop a customized approach in response to the needs of the population and resources of the program. The material presented in this toolbox is also adaptable to other populations and other chronic diseases.
